Before your first visit with us, we ask that you take a few minutes to fill out the necessary forms. There is a Health History which helps us to understand what your wants and needs are.
When you arrive for your first visit, you can expect a warm and friendly welcome. Your initial visit will last approximately one hour. We will complete a full comprehensive exam which includes a full series of digital x-rays, a series of pictures, and a periodontal evaluation to determine that type of cleaning is necessary. Once your exam is complete, together you and Dr. Perry will develop a treatment plan.
